So true @Matt, makes me cry too. I needed a full custom with about 40mm more stack, but the reach was correct for what I needed. Couldn't afford a full custom, so beggars can't be choosers.
Who knows, as I age, I may be able to get comfortably to a more aero position and look really cool. We can only hope.

However, to make @Matt 's tears dry up, I may someday drop the stem rise, and instead raise the armpads and extensions independently from the bullhorn with the profile design pad riser kit(s). This may look cooler, and maintain my position. It will give that large separation you see on some TT bikes between the pads and the bullhorns.
It would look cooler, but I'm not that worried or vain to make all that effort yet.
However, we have to represent the team proudly, so I may do it simply for that reason.

The turbo doesn't really do the bike justice (nor does the lurid lycra clad imbecile on top) but to satisfy interim bike porn needs here's a photo I (aka my girlfriend) took whilst I was playing about with fit....

@ellsworth53t by forward you mean forward down the road yes? It already feels like I'm reaching a touch but that might be a need to pull the extensions a bit shorter... Hmmmm
It does look like you also want to shorten the length of the extension, it might help with that feeling of reaching too far. Your index finger looks extended out to the shifter, but you probably want the shifter closer to the middle of your finger.

Stem fixed. I ride it this way for sprints, and 30mm stock profile risers for just the pads/extensions for long course.
